Con Safos BJJ, LLC
RELEASE OF LIABILITY, WAIVER OF CLAIMS, AND ASSUMPTION OF RISKS AGREEMENT
I understand and acknowledge that participation in Brazilian Jiu-Jitsu classes, conducted by Con Safos
BJJ, LLC, involves risks and hazards that could result in injury, illness, or death. These risks and hazards
include, but are not limited to, bodily injury, broken bones, sprains, strains, cuts, bruises, and other
injuries.
I further understand that the activities involved in Brazilian Jiu-Jitsu, including grappling, are inherently
dangerous and involve a risk of serious injury, including but not limited to, brain damage, paralysis, or
even death.
In consideration of being allowed to participate in Brazilian Jiu-Jitsu classes conducted by Con Safos BJJ,
LLC, I hereby agree to assume all risks and hazards associated with participation in this activity, including
those mentioned above, and to waive, release, and discharge Con Safos BJJ, LLC, its owners,
employees, and agents from any and all liability for any injuries or damages that may arise from my
participation in this activity, including injuries associated with any form of grappling.
I further agree to indemnify and hold Con Safos BJJ, LLC, its owners, employees, and agents harmless
from any and all claims, damages, or expenses that may arise as a result of my participation in this
activity.
I understand that Brazilian Jiu-Jitsu classes are open to participants of all ages, and as a parent or legal
guardian, I give my consent for my child to participate in these classes.
I hereby agree to assume all risks and hazards associated with my child's participation in Brazilian
Jiu-Jitsu classes, and to waive, release, and discharge Con Safos BJJ, LLC, its owners, employees, and
agents from any and all liability for any injuries or damages that may arise from my child's participation in
this activity, including injuries associated with any form of grappling.
I further agree to indemnify and hold Con Safos BJJ, LLC, its owners, employees, and agents harmless
from any and all claims, damages, or expenses that may arise as a result of my child's participation in this
activity.
I have read this release of liability, waiver of claims, and assumption of risks agreement, and fully
understand its contents. I acknowledge that I have had the opportunity to ask any questions about the
activity and the risks associated with it.
